Liability Release, Assumption of Risk, and Hold Harmless Agreement – State of Florida
This Liability Waiver (“Agreement”) is entered into by the undersigned participant (“Participant”) in consideration for being permitted to attend, observe, or participate in any training, seminar, class, or event held by Lawman’s Black Belt Academy, LLC (“Company”).
1. Assumption of Risk
Participant understands that defensive tactics, grappling, and ground control training involve inherent risks including, but not limited to, strains, sprains, bruises, joint injuries, falls, and other physical injury. By checking the consent box below, the participant voluntarily assumes all risks associated with participation.
2. Release and Hold Harmless
To the fullest extent permitted by Florida law, Participant releases and discharges Lawman’s Black Belt Academy, LLC, its instructors, agents, and affiliates from any and all claims, demands, or liability arising from participation in training activities, whether caused by negligence or otherwise.
3. Emergency Contact Reference
Emergency contact details, including name, relationship, phone number, and agency/organization, are provided in Section III of the Registration Form and will be used in case of emergency.
Part III: Final Declaration and Consent
I certify that the information provided in this registration is true and complete to the best of my knowledge. I understand that any false statements or omissions are grounds for immediate disqualification or dismissal from the program.
